چکیده

TREATMENT of chronic pain in patients with a history of opioid addiction is difficult since tolerance to the opioid agonist effects may require extremely large opioid doses for pain control. Furthermore, insufficient pain control may be hard to discern from craving for more opioids. Although drug-free periods, also called “drug holiday,” can reestablish sensitivity to drugs used to treat Parkinson disease and schizophrenia this strategy has not been described for patients with chronic pain presenting with tolerance to opioids. We report an opioid addicted patient with intractable cancer pain during methadone substitution therapy in whom antagonist supported opioid detoxification and a drug holiday reestablished sensitivity to opioids and improved cancer pain.

Very low dose naltrexone addition in opioid detoxification: a randomized, controlled trial.

Although current treatments for opioid detoxification are not always effective, medical detoxification remains a required step before long-term interventions. The use of opioid antagonist medications to improve detoxification has produced inconsistent results. Very low dose naltrexone (VLNTX) was recently found to reduce opioid tolerance and dependence in animal and clinical studies. We decided...

Chronic mu-opioid receptor stimulation in humans decreases muscle sympathetic nerve activity.

BACKGROUND Opioid-addicted patients undergoing detoxification provide a unique opportunity to assess the effects of chronic opioid receptor stimulation on the sympathetic nervous system. We tested the hypothesis that chronic oral methadone intake decreases resting efferent sympathetic nerve activity to muscle (MSA). Furthermore, we assessed whether this effect is reversed by mu-opioid receptor ...

Anesthesia-assisted vs buprenorphine- or clonidine-assisted heroin detoxification and naltrexone induction: a randomized trial.

CONTEXT Rapid opioid detoxification with opioid antagonist induction using general anesthesia has emerged as an expensive, potentially dangerous, unproven approach to treat opioid dependence. OBJECTIVE To determine how anesthesia-assisted detoxification with rapid antagonist induction for heroin dependence compared with 2 alternative detoxification and antagonist induction methods. DESIGN, ...

Comparison of the Efficacy of Methadone and Tramadol in Opioid-Assisted Detoxification

Background: A number of drugs are used to control opioid withdrawal symptoms during detoxification. Tramadol, as a partial opioid agonist, has been widely used to manage acute and chronic pains. The present study was designed to assess the effectiveness of tramadol in methadone-assisted detoxification. Methods: In a double-blind randomized clinical trial 72 opioid-dependent patients as defined ...

Impact of Transcranial Direct Current Stimulation on Ultra-Rapid Opioid Detoxification Outcome

Introduction: The persistence of post-detoxification problems in drug addiction is one of the disadvantages of the ultra-rapid opioid detoxification (UROD) method. Transcranial direct current stimulation (tDCS) has been introduced in experimental addiction treatment for some years. Results of pilot studies suggest that it might be a promising method for addiction treatment. This study explores ...
